NHS Referrals
We accept NHS referrals from a wide range of professionals including health professionals, social workers and care providers.
If you wish to make a referral, please complete the following form and we will contact the patient within 24 hours to arrange their Telecare alarm. It is easy to make a referral for your patient, client, or service user. Simply fill out the referral form and our friendly team will handle the rest.

If you know of someone who would benefit from having a personal alarm to gain back their independence, you can also refer them to us by completing this form. Each successful referral will receive a £15 One4All voucher.
Why should you get a Telecare alarm?
Telecare alarms are a failsafe way to ensure that patients being discharged from hospital are well protected once they return to their own homes.
After being discharged, many elderly or disabled people feel more vulnerable than they did before their hospital stay. This can be due to new medications, ongoing recovery from an illness or simply feeling more anxious than before. Health professionals can also worry that patients will not be able to cope with independent living once they return home.
A Telecare alarm can ease the tension in this situation by providing a level of support, a safety net, to a patient who is returning home. Every alarm user receives a Telecare pendant that can be used at any time to call for help. Once our 24-hour monitoring centre receives the call, our team will contact family, friends, carers or 999 to ensure the patient is safe and well.
